Do we take your insurance?


The answer is YES! We work with almost all dental insurance companies and we are preferred providers with Delta Dental, Regence, Premera and Cigna. We have done our best to be in -network providers for some of the biggest employers here in the Seattle Metropolitan Area. A list of some of those companies can be found below. If you have any questions do not hesitate to contact our office and our patient care coordinators will assist you with all of your insurance related inquiries.

Queen Anne Family Dental Appointment Protocol


The doctors and team members of Queen Anne Family Dental are dedicated to providing the highest quality of dental care to you and to all of our patients. We value our relationship with you and our promise is to stay at the forefront of clinical excellence, patient care and services. We would like to take this opportunity to explain the rationale of our Appointment Protocol. When a patient either fails to show for an appointment, or changes an appointment on short notice, many things happen; Your treatment is delayed, which in some cases can further complicate your
conditions. Another patient waiting for care cannot be seen because we/they didn't have sufficient time to make the necessary arrangements to be seen. We do understand that occasionally situations may arise which may require rescheduling your appointment with us.

However, due to the reasons listed above, we do ask that you inform us of any necessary changes two business days prior to your appointment. For those patients with a pattern of short notice cancellations and/or no-shows, pre-payment for your appointment or an appointment no-show fee will be charged at the discretion of our team. If you have any questions regarding our appointment protocol, please feel free to ask any member of our team. We will be happy to assist you.

Queen Anne Family Dental Financial Policies


We are happy to have you as our patient and look forward to offering you and your family the finest dental care available. We know that providing complete comprehensive dental services includes discussing all treatment and financial information. Before treatment is performed, we will discuss treatment and financial options. This will allow you to fully understand your dental treatment, what to anticipate in fees and allow you time to make the necessary financial arrangements. Payment is due on the date of service unless prior arrangements have been made. For your convenience we accept cash, checks and credit card. We reserve the right to charge a $35.00 fee on all returned checks.

Dental Insurance: In order for us to maintain a high level of service to you, we provide the courtesy of submitting your insurance claim on your behalf and supporting you by maximizing your benefits. Insurance is not a guarantee of payment; it often does not cover all the costs involved in treatment. You will be expected to pay for services rendered if this office is unable to verify your insurance information before treatment. Any deductible or estimated co-payment amount will be due at the time of treatment. Policy coverage, changes, and follow-up on unpaid claims is your responsibility. We will make every effort to provide the most accurate estimate of your covered benefits but please be aware that all estimates presented and signed are just that, an estimate. Please be prepared to show your insurance card at the time of your first visit. If your insurance company has not made a payment within 60 days of billing, the balance will become your responsibility. Insurance coverage is a contractual agreement between the insurance company and you or your employer. We have no control over this relationship.

Delinquent Accounts: After 90 days, all accounts that are not paid in full may be sent to a third party collection agency. Any accounts turned over to collections will be assessed a collection fee of 33%.
